Marijuana use may increase risk of testicular cancer

A new study from the University of Southern California (USC) has found a link between recreational marijuana use and an increased risk of developing subtypes of testicular cancer that tend to carry a somewhat worse prognosis. Published early online in CANCER, a peer-reviewed journal of the American Cancer Society, the findings suggest that the potential cancer-causing effects of marijuana on testicular cells should be considered not only in personal decisions regarding recreational drug use, but also when marijuana and its derivatives are used for therapeutic purposes in young male patients.

Testicular cancer is the most common cancer diagnosed in young men ages 15 to 45 years. The malignancy is becoming more common, and researchers suspect this is due to increasing exposure to unrecognized environmental causes.

To see if recreational drug use might play a role, Victoria Cortessis, MSPH, PhD, assistant professor of preventive medicine at the Keck School of Medicine of USC in Los Angeles, and her colleagues looked at the self-reported history of recreational drug use in 163 young men diagnosed with testicular cancer and compared it with that of 292 healthy men of the same age and race/ethnicity.

The investigators found that men with a history of using marijuana were twice as likely to have subtypes of testicular cancer called non-seminoma and mixed germ cell tumors. These tumors usually occur in younger men and carry a somewhat worse prognosis than the seminoma subtype. The study's findings confirm those from two previous reports in CANCER on a potential link between marijuana use and testicular cancer.

"We do not know what marijuana triggers in the testis that may lead to carcinogenesis, although we speculate that it may be acting through the endocannabinoid system?the cellular network that responds to the active ingredient in marijuana?since this system has been shown to be important in the formation of sperm," said Cortessis.

The researchers also discovered that men with a history of using cocaine had a reduced risk of both subtypes of testicular cancer. This finding suggests that men with testicular cancer are not simply more willing to report a history of using recreational drugs. While it is unknown how cocaine may influence testicular cancer risk, the authors suspect that the drug may kill sperm-producing germ cells since it has this effect on experimental animals.

"If this is correct, then 'prevention' would come at a high price," Cortessis said. "Although germ cells can not develop cancer if they are first destroyed, fertility would also be impaired. Since this is the first study in which an association between cocaine use and lower testis cancer risk is noted, additional epidemiological studies are needed to validate the results."

This is Chris Terry, speaking from beyond the digital grave. This post is left over from my tenure as?the 2011-?12 Graduate Student Ambassador for the Creative Writing ? Fiction MFA program. The folks at the grad office were kind enough to run it for me.

Eli, AKA Eliza, and I started the program at the same time. She was one of the people whose work I?d always anticipate hearing when we?d share what we?d just written in class. Here?s an excerpt from one of her thesis stories, written from two different perspectives, like we do in our Steeplechase assignments. The 1st person version of ?Count Me? was written for Don DeGrazia?s Thesis Development class, while the 3rd was written for Mort Castle?s Writing Horror Class.

1st??

NIGHT wind. Tracy?s black Trans Am. A purple sky full of August stars. As the car slows into a curve, the beams dip to the side and light up the trees. The bark has given up and started peeling, covering the trunks in papery curls. We can only see as far as the headlights, pieces of forest and road that look lonely and black-and-white. ??Make a wish,? I say to Tracy and Katie, taking a deep breath and counting to ten. The air tastes like boiled pine needles, like it did in summer camp when we used to sit around a teepee-shaped fire waiting for stars to fall into our laps.

?Wish for what?? Tracy says with one hand on the wheel and the other checking her lipstick in the rearview mirror.

?Yeah. Drop-dead,? Katie says and blinks, ta-ta, with her long lashes, rolling her eyes and laughing as she sinks into the backseat.

I don?t know what to say. I don?t have a quick biting jab so I laugh, too, ha-ha. The dashboard light spills across our teeth and turns them into chips of sea glass.

We?re headed past the deadlight glow of the CITGO, toward the fairgrounds lined with vintage cars and splashy carnival lights. We?re speeding into the last days of summer so fast we could be one of the sparkly rides, maybe a Sizzling Zipper or a Jumping Star.

Tonight, we?ve put away our high socks and bubble gum and loosened our French braids. We?ve slid into lilac tube tops and iced our eyelids into frosty cakes. Katie?s rubbed pink-diamond dust on our cheeks and sprayed her favorite perfume on our necks. The scent is too sweet and boozy for Tracy and me, but we do what Katie says. She tells us?where to sit, what pills to pop, where to party, how much vodka to pour into our grape sodas, how to honey our hair into summer gold, how to lie to our mothers, how to tuck in our nightshirts and sneak out our windows because the best things always happen after midnight-lights out?and we always listen.

This happened somewhere once; the year Tracy, Katie and Phoebe turned nine. The same year the gingerbread house disappeared. Though, no one could remember or agree that they?d ever seen such a structure in La Porte. The window ledges and eaves glittered with snow?even in August. The sparkling dust was actually sweet porridge and pine needles. The gruel glistened because, as Phoebe explained, the sun warms the milk into a layer of sugary skin.

A path of cobbled stone steps led to a one and a half story home with two pitched roofs. Tracy remembered a smaller hall set in front of a larger one. Katie said the walls were the color of a burlap sack. Phoebe told her therapist about the lights hugging the front door, describing the yellow orbs with words like amber and honey-ish. ?They looked like crinkly wrappers from a giant?s stash of butterscotch candies,? she told Dr. Almond. He sucked on the end of his pipe, deepening the brown tint on his white moustache. He scribbled Active imagination on his notepad.

The girls insisted upon Cray-pas to sketch their drawings because they wanted precise colors, mixing oil pastels for a palette with names like pistachio green, peach kitten and buzzard-blue twilight. Any person older than sixteen would look at the pictures of heart-shaped windows filled with stained glass mosaics, quietly count the crudely stacked stones of the undulating chimney, turn the picture upside down to see if those were really stars beneath the house?s tendrilous roots, and push the drawings away, head-shaking and mumbling to the floor?It?s not possible. It?s never been.

It seemed unlikely that such a house ever existed or that it could ever be built due to practical zoning codes. Besides, the depictions showed a house that had fought its way up from the dark ground, an extension of the earth, a building that had always existed and the rest of the town and the woods were an afterthought.

Somewhere, once, on the same August night the stars turned violet, Tracy, Katie and Phoebe slipped away from Brownie Troupe #11?s golden bonfire and found themselves lost and alone in the woods. Their story has been told again and again?even though it happened just once. There?s only fear in speculating everything that could?ve eaten those little girls that night. Wolves, perhaps because they never give up, having survived the dark forests and the lore of fairytales. The wolves live on, yes, but the same is never said about the children.

I spent a great deal of yesterday deeply distracted, profoundly stressed and extremely frustrated. The problem was getting my medical records transferred from Oregon Health Sciences University (OHSU), my ?home? hospital, where I am treated at the Knight Cancer Institute, to the Johns Hopkins Hospital (JHH), where my second opinion consults today and Monday are at the Kimmel Cancer Center.

That turned out to be an absolute nightmare.

Bear in mind that I have very good health coverage (in terms of US health insurance). I am a capable businessperson who takes good call notes, knows how to speak professionally to people even when I am under extreme stress, and has long experience in navigating complex corporate systems and policies, as well as specifically with being a cancer patient. In other words, I am well supplied with experience and skills to deal with this.

The problem appears to be systemic rather than any individual misperformance. Nonetheless, this was a comedy of errors which rapidly approached a deep problem for me as I prepared for my second opinion consultation with a medical oncologist at JHH. It went something like this:

On August 17th, I spoke to a staff member in OHSU?s Medical Correspondece office, that handles patient records transfers. I requested release of my records to JHH. I detailed the treating physicians, as I have had four of them over 4.5 years of living with Stage IV colon cancer with multiple metastatic presentations. I was assured they had all the information they needed, and everything would be taken care of.

On September 5th, i spoke to a staff member at the Kimmel Cancer Center (they are a patient intake specialist) to confirm all required material had been received. After assuring me that if anything were missing they would have let me know, they double checked at my insistence. JHH had case notes from one of my doctors, nothing from the other three.

That same day I called back and spoke to Medical Correspondence at OHSU again. Asked them to resend for my other three doctors.

Late in the afternoon I tried unsuccessfully to reach my contact at JHH to confirm receipt.

Yesterday morning, September 6th, I finally reached JHH, who informed me that they had received partial notes from a second doctor, and none from the remaining two.

I called back to OHSU Medical Correspondence and spoke to another staff member. They quite incidentally asked me which month this year I?d been seen in by these other doctors whose records were missing. When I said these records went back to 2008, the staffer was surprised. They then said they would take care of it.

OHSU then called me back to tell me that they could not get the faxes to go through to JHH. Something seemed to be wrong with the receiving fax machine.

I then spent several hours trying to coordinate communication directly between OHSU and JHH rather than playing operator to a series of failed communication attempts. I offered to have the records faxed to my virtual fax number, where I would receive them as .pdf files and could forward them on to JHH via email, but was told that individuals cannot receive patient record faxes. Not even me for my own records. (This is in direct contravention to both stated OHSU policy and Oregon state law, by the way.)

At that point, I was in a panic. If the medical records, including surgery reports and case notes, did not go through, my entire trip to Baltimore for the second opinion would be largely wasted.

As the day went on, I made and received another half dozen phone calls. OHSU and JHH had finally spoken directly. A new fax number was provided. Another incomplete fax was sent. I was forced again to mediate between the two hospitals. Eventually, all the records went through, though receiving even confirmation of that was difficult.

Some observations about all this, speaking in my professional capacity as a business process and business communications analyst:

  1. OHSU?s process is broken. When I made my initial records request I was never asked for the date range of treatment. In assuming same-year only, OHSU creates a problem for patients with long-term courses of treatment. I did not know to inquire about the date ranges, and it?s not an obvious question for a layman to think to raise.
  2. OHSU?s process is further broken in that they cannot detect when they have sent an incomplete fax transmission. As delivered page count is readily available within facsimile transmission protocols, this means there?s no functional monitoring in place.
  3. JHH?s process is broken in that their error checking with respect to records availability failed. Further, that error check failure was not noticed until I pushed the issue.
  4. JHH?s process is further broken in that their fax machine (or more likely, electronic fax board) associated with such a critical path function was defective, apparently without anyone noticing it. This means there?s no functional monitoring in place.
  5. As a patient, it is virtually impossible for me to facilitate direct communication between OHSU and JHH due to the widespread use of voicemail boxes and callbacks. This meant I had to play ?operator? and lose several hours or half a day or more to each failure point in the process.
  6. OHSU?s lack of willingness to directly send me my own records in contravention to both their own published policies and Oregon state law made a swift, simple solution to the problem impossible. Another process failure, and possibly an audit and accountability failure within OHSU?s own organization.
  7. The failure of both institutions to either properly implement the 30-year-old technology of fax machines and transmission monitoring, or to implement in any way the 20-year-old technology of electronic records transmission, which in either case would sidestep many of these legacy issues and significantly reduce the latency in the records transmission process, allowing swifter error detection and prompt resolution.

I was almost in the position of having wasted an enormous amount of time and money trying to buy myself an extra year or two of life with outside experts consulting on my late stage cancer, because OHSU could not fax a set of documents to JHH. Neither entity was capable of performing simple, proven error detection at the functional telecommunications layer. Neither entity was capable of performing simple error correction at the business layer on their own initiative. Resolving this situation required well over a dozen intervening phone calls from me personally, at a time when I am already very, very stressed by the fourth presentation of my cancer.

Someone without my business and communication skills would have been at a dead loss, and found their trip for the second opinion wasted. Someone with my skills who simply hadn?t followed up in the absence of news to the good or bad would have likewise been at a dead loss, trip wasted.

And this is merely one small part of the incredibly complex process that I have to go through as a cancer patient. This is what our medical system asks of people in their times of deepest need and highest stress. Finicky compliance, constant vigilance, and that the patient or their family have the freedom and flexibility and cognitive functionality to cope with technically trivial and highly avoidable process errors.

If you think our American healthcare system is the best in the world, try being seriously ill. While I have only very minor complaints about my quality of care at the delivery end, the business, administrative and financial wrappers around that delivery are onerous, punitive and bizarre, on their best days.

How do people who don?t have advanced educations, business experience and good communications skills do this? How many people die every year in this country because they simply can?t comply with the Byzantine requirements of the healthcare system? Do you seriously believe we don?t need healthcare reform in the United States? My experience is not an exception case, it is the norm, even for someone with good insurance coverage and strong life skills.

It?s bad enough that I?m sick with a disease that will almost certainly kill me. What I have to go through to receive treatment is a disgrace. And I am one of the lucky ones at that,

What does your company/ business do? We are the regional development program representative of Dairy Australia within the Subtropical Dairy region, which commences at Kempsey, NSW in the south through to the atherton Tablelands in the north.

Describe your job on the average day There is no such thing as an ?average day? as they vary from office/ administration roles of normal business and financial operations to project meetings, on farm workshops/ field days/ seminars as well as phone calls, emails, teleconferences and webinars on a large variety of topics.

How did you get involved with agriculture? My extended family was involved in agriculture and then through various studies and jobs are found myself working within an agricultural environment.

Education I complete Senior and subsequently have undertaken studies across a range of disciplines including Education and Training, Agriculture, Marketing, Business Management, Project Management and Rural Business Management. I am currently completing a Cert IV in TAE as well as a Graduate Diploma in Facilitation.

If you studied/ trained in agriculture, why did you choose that degree/ certificate? The agricultural studies have been through the VET sector (vocational and educational training). I studied later in my career to ensure that I am up to date as possible with the latest issues and technologies available to farmers.

What are/were you plans after school/TAFE/university? Realistically I was unsure as to my career direction after school and tried a number of different roles but all seemed to gravitate back to agriculture, which is where I now intend to stay.

Being able to produce a consistent quality product that will meet the needs of the community in a cost effective and efficient manner, whilst still providing a reasonable financial return on their investment.

Why do you think less people are becoming involved in agriculture? The perceived lack of a profitable and desired business outcome which will provide for future generations

What advice do you have for people thinking about getting into agriculture? There are a plethora of agribusiness/ agricultural associated jobs and roles available so you need to speak to people in the industry to gather a range of opinions and options about how you can be involved in the broader agricultural industry. Plan ahead and consider a range of differing roles of a generalist nature before deciding to specialise later.

How important do you think an agricultural background is to become involved in the industry? It is a help but not essential, as sometimes the best ideas come from other sources. On the other hand an agricultural background can give you ?baggage? with some preconceptions and experiences that could influence your future thinking.

What do you think is the most common misconception about agriculture? That the only job is as a ?farmer? and that you cannot make a good living from being ?just a farmer? and the saddest thing is that this information often comes from farmers talking their own industry down.
